Qualifications: 2 years of experience in long term care, licensed social worker or bachelor’s degree preferred, must possess excellent communication and organizational skills As a Director of Social Services in a Grace Management Services facility you must be able to perform the following essential functions as demonstrated by the ability to: Essential Job Functions: Develop one-to-one relationships with residents and families.
Interact with residents and families dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.
Establish relationships with area hospice agencies.
Complete the social service portion of the MDS within 14 days of admission.
Document the social care component of the Comprehensive Care Plan for each resident as identified by the interdisciplinary team within 7 days of completion of the MDS.
Document all interactions with resident and/or family in the assessment and care plan as required by state standards.
Document the social and emotional needs of residents.
Participate in utilization review meetings.
Act as a liaison with social, health and community agencies.
Be responsible for referral of residents to social, health and community agencies and ensure documentation in the resident’s record concerning the result of such referrals.
Coordinate family and community resources as needed to solve financial needs and to promote emotional security.
Take part in in-service education and attend all in-service sessions relating to the Social Service Director.
Participate in admission of residents as assigned.
Coordinate with Activity Department to meet socialization needs of the residents.
Oversee coordination of referrals for ancillary services (dentist, eye doctor, podiatry, psychology, psychiatry, and audiology).
Assist in admissions and discharge process as needed.
Promotes a positive image of Sprenger Health Care Systems at all times.
Participate in plan of care meetings.
Maintain log of misplaced property and ensure follow-up.
Participate in Resident’s Council meetings at least once a month.
Must be able to speak in public and in front of large groups.
Clear written and verbal communication skills.
Proficiency in Word and Excel.
Good time management skills.
Works well in a team environment.
Ability to maintain confidential information.
Perform other related duties as directed by the Administrator.

Must be able to balance, climb, crouch, grasp, kneel, pull, push, reach, stand and stoop adequately to perform the essential functions of the job.
Must be able to hear, smell, feel and speak adequately to perform the essential functions of the job.
Must be able to read and write the English language.
Must be able to work inside and outside with exposure to outdoor weather conditions.
