Job Title: Licensed Practical Nurse

Summary:
As a partner in building brighter futures for youth and their families, your role will be to provide medical care to our youth and other medical compliant activities that align with The Village Network’s Mission, Vision & Values.
Essential Tasks, Duties, and Responsibilities:
Meets with each client prior to each psychiatric appointment to gather information such as pulse, blood pressure, and medication experiences, etc.
that will assist the prescriber in meeting the clients’ needs.
Communicates to the prescriber the information gathered prior to the psychiatric appointment.
Provides Medication counseling and education under the leadership of the RN.
Organizes and Monitors Medication Record Sheets (MARS) in residential programs, under the leadership of the RN.
Calls pharmacies for prescriptions as directed by physician or Advanced Practice Registered Nurse (APRN).
Maintains adequate documentation of nursing activities in Medical Record.
Participate in on-call duties/rotation based on needs of clients.
Other reasonable tasks within the scope of practice assigned by the RN.
Knowledge, Skills, and Abilities:
Ability to use a computer; proficiency with Microsoft Office products and CATT.
Must be licensed in Ohio as a Licensed Practical Nurse (LPN) with 2 years of previous relevant experience.
Excellent verbal and written communication skills; previous experience with adolescent population and psychiatric medicine helpful; willingness to be flexible is necessary.
Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance.
Physical Demands:
Occasionally move about inside the office to access file cabinets, office machinery, etc.
Constantly operates a computer and other office productivity machinery, such as a copy machine, computer printer, etc.
Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
Must be able to lift 20 pounds independently
