Job Summary:
The Social Worker fosters patient autonomy by providing patients with information that enhances their ability to make appropriate health care decisions and/or receive medical care with an enhanced sense of confidence when making healthcare decisions.
The care manager provides individual guidance, education, and other assistance to beneficiaries as they move through the healthcare continuum developing care plans as a guide.
The Social Worker provides guidance and help accessing needed resources.
As part of a collaborative team, the Social Worker will be responsible for assessments and the development of care plans.
Essential Functions:
1.
Maintain efficient and timely clinical documentation.
2.
Complies with all HIPAA and OSHA regulations and policies.
3.
Responsible for working with members of the care team and assist in the coordination of care of selected beneficiaries.
4.
Provides one on one guidance, support, education, referrals, coordination of care and other assistance to patients as they move through the health care system.
5.
Provide behavioral health treatment.
6.
Participate in scheduled clinical meetings.
7.
Build relationships with providers, home health agencies, hospitals, nursing homes, DME providers etc.
8.
Provide community service information as an outreach professional as required.
9.
Assist with the scheduling of appointments and transportation when necessary.
10.
Assume other responsibilities as directed.
Competencies:

Experience in outpatient mental health treatment.
2.
Open to all specialized areas of interest.
3.
Ability to assume an independent caseload.
4.
Capable of working with diverse social, racial and ethnic groups and collaborate with professionals from many different social service organizations and government agencies.
5.
Ability to prioritize and plan work activities, use time efficiently and develop realistic action plans.
6.
Comfortable with use of computers, capable of effectively utilizing word and excel programs.
7.
Ability to identify and resolve problems in a timely manner and gather as well as analyze information skillfully.
8.
Ability to adapt to changes in the work environment, manage competing demands and ability to deal with frequent change, delays or unexpected events.
9.
Demonstrates dependability by being consistently at work and on time, follows instructions, responds to management direction and solicits feedback to improve performance.
10.
Demonstrates accuracy, thoroughness, and monitors own work to ensure quality.
11.
High energy level, comfortable performing multifaceted projects in conjunction with day-to-day activities.
12.
Ability to communicate effectively in written and verbal form.
13.
Use of personal vehicle for home visits.
Education and Work Experience:
1.
Associates or Bachelor’s degree in social work from an accredited educational institution
2.
Minimum of two years’ experience in health care setting preferred.
Certificates and Licenses Required:
1.
Licensed in Social Work in the State of Ohio.
Environmental/Working Conditions:
Office setting, well-ventilated and well lighted.
Work involves frequent telephone contact with patients.
Work may be stressful at times.
Interaction with others is constant and interruptive.
Physical/Mental Demands:
Normal office environment.
Work is typically performed at a desk or table requiring the use of standard office equipment.
Intermittent sitting, standing and stooping.
May view computer screen for long periods of time.
